What is MyChart?

MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to:

  • Request medical appointments.
  • View your health summary from the MyChart electronic health record.
  • View test results.
  • Request prescription renewals.
  • Access trusted health information resources.
  • Communicate electronically and securely with your medical care team.

Is there a fee to use MyChart?

MyChart is a free service offered to Saint Francis Health System patients.

What do I need to use MyChart?

You need access to a computer connected to the Internet and an up-to-date browser. You can also access it through an Apple or Android device.

How do I sign up?

You can sign up three different ways. No matter which way you choose, you will need an activation code. This code will enable you to login and create a personal user ID and password. Please choose the method that works best for you. 1. Sign up in-person with your medical care team during an office or hospital visit by asking for the activation code. 2. Call your Warren Clinic physician’s office to request that an activation code be sent to you. 3. Request an activation code online from www.saintfrancis.com/mychart.

Is MyChart secure?

Access to information is controlled through secure activation codes, personal IDs and passwords. Each person controls their password, and the account cannot be accessed without that password. MyChart uses the latest encryption technology to secure the integrity of your account. Unlike conventional email, all MyChart messaging is done while you are securely logged on to the website.

Which physicians may I make appointments with in MyChart?

Appointment requests can be submitted for Warren or Laureate Clinic providers where you have had an appointment within the past year. Direct scheduling through MyChart is available from your Warren Clinic primary care physician. Appointments with a new physician may be made through referral, by calling your physician’s office or by calling the Warren Clinic Physician Referral Line at 918-488-6688.

When can I see my test results in MyChart?

Test results will automatically release within one to four days of their being finalized. Tests resulted during your inpatient stay will be released at 24 hours, after your discharge. Those tests resulted after the inpatient stay will follow the automatic release previously described.

If I send a message to my physician or nurse, when can I expect a reply?

You will generally receive an answer within 1–3 business days. Please note that MyChart should not be used for urgent situations. Please dial 911 if it is an emergency.

Will my entire medical history be made available in MyChart?

MyChart is a direct link between you and the information in your electronic medical record. It will not reflect your complete medical history. Any appointment that has occurred in our new electronic medical record can be viewed under Appointments > Past Appointments.

Can I ask my primary care physician for medical advice?

Yes. You can send a secure message to your Warren Clinic primary care physician.

Can I view a family member’s health record in MyChart?

Yes, you can. This is called proxy access and allows a parent (or legal guardian) to log into their personal MyChart account, and then connect to information regarding their family member. Complete and submit one of the following forms to request access to this convenient service.

To access the records for your child who has had services at Saint Francis Health System, you must complete a MyChart Child proxy form.

If you are an approved caregiver for another adult, such as elderly family member, an Adult proxy form is required.

Can I ask questions regarding a family member from my MyChart account?

MyChart offers direct access to your personal health record. If you asked a question about a family member, the response would appear in your health record and not in the family member’s health record, potentially jeopardizing your family member’s medical care. We recommend asking questions only about the patient who is currently logged in to MyChart.

Can my spouse and I share one MyChart account?

No, each adult must establish their own MyChart account.

If some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record at your physician’s office. Contact your physician’s office to correct any inaccurate information. If the information in question is related to a hospital department service, contact the medical records department at Saint Francis Health System.

Is my activation code my user ID?

No, your activation code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.

Can you send me a new activation code as I have lost it or did not receive it?

Please contact your Warren Clinic physician’s office to notify them. After we verify your information, a code will be sent to you.

Where can I update my personal information (e.g., home address, email or change my password)?

Log into MyChart and go to the Preferences section and select the appropriate option.

I forgot my password. What should I do?

Visit the Saint Francis MyChart site and click the Forgot Password link on the sign-in page to reset your password online.

What is MyChart’s Privacy Policy?

MyChart is fully compliant with federal and state laws pertaining to your privacy. Your name and email address will be treated with the same care and privacy given to your health records.
