MyChart Agreement, Terms and Conditions

Saint Francis Health System, referred together with its affiliated hospitals, clinics and physicians who utilize Saint Francis Health System's electronic medical records system, provides you with access to MyChart. MyChart is a secure, web-based service that lets patients, or another person chosen by the patient or permitted users (a "proxy"), to have secure web-based access to parts of their electronic medical record. Use of MyChart by authorized persons is completely voluntary. You must have an email address and be willing to use MyChart exclusively (i.e. not let others have your password to use MyChart).

MyChart includes selected information from your providers of health care across the integrated Saint Francis Health System ("Saint Francis"), such as prescriptions, test results, summary medical history and appointments.

Security, Confidentiality and Usage Guidelines for MyChart

  • MyChart is designed as a secure internet-based record where you may receive confidential medical information about yourself.
  • Once you sign up for MyChart, you will be given a one-time only code that you will use to start your account.
  • You will then create your own MyChart USERNAME, PASSWORD and PASSWORD hint.

Password Guidelines

  • DO NOT SHARE your password with anyone. The MyChart USERNAME and PASSWORD you choose when you first log-on to MyChart are the way for you to protect the confidentially of your information.
  • Your password should be unique and not the same password used for other applications; you are responsible for remembering your MyChart USERNAME and PASSWORD and for keeping them confidential.
  • You should write down your MyChart USERNAME and PASSWORD hint and keep them in a safe place.

Activation Guidelines

  • Once you have activated your MyChart account, MyChart will become the main way for you to get important messages from your doctor, like selected test results and other personal medical information. You will still receive communications via other means.
  • You MUST provide us with your email address when activating your MyChart account and keep your email address current in MyChart.
  • You need to make sure your email address is up to date with Saint Francis MyChart so that you will be notified about new information right away.

Communication Process

  • Check your email on a regular basis for MyChart communications.
  • No confidential information will be sent to your email, but you will receive email notification whenever a new result has been released to your MyChart account.
  • You will receive your personal health information from your doctor in your personal MyChart account.
  • Please never send any messages requiring urgent attention using MyChart.

Urgent Medical Matters

For all urgent medical matters, contact your doctor's office, go to an emergency room, or call 911; for all medical emergencies, immediately dial 911.

MyChart User Responsibilities

By using MyChart you agree to be bound by all the terms and conditions of this Agreement and our Privacy Policy. Please read this Agreement and Privacy Policy periodically as we may change them at any time. By enrolling in MyChart, you agree that any changes will affect you as soon as they are posted.

MyChart is a communication service offered as a convenience to patients of Saint Francis. You understand that it provides you with online access to parts of your Saint Francis electronic medical record, and that you are solely responsible for any sharing of MyChart content that you communicate to others or that others may obtain by any means using your MyChart username and password.

You will promptly notify us of any unauthorized use of your MyChart account or password – or if you learn of any other security breach related to MyChart.

You agree that you will not communicate any offensive material or language on this site or otherwise through the MyChart service. If we determine that you have violated this Agreement or abused the use of this service, we reserve the right, at our sole discretion, to stop your use of the MyChart service. You will be notified if your MyChart service is discontinued.

You agree that you shall not impersonate any other person; use any portion of MyChart as a destination linked from any unsolicited bulk messages or commercial messages ("spam"); use any automated process or service (such as a BOT, a spider, periodic caching of information or "meta-searching") to access and/or use MyChart; use any unauthorized means to modify or reroute MyChart; or damage, disable, overburden, or impair MyChart (or the network(s) connected to MyChart) or interfere with anyone's use of MyChart.

You can discontinue your use of MyChart at any time by contacting your physician's office.

Privacy Policy

Saint Francis understands the importance of protecting the confidentially of your medical information. To that end, we have detailed policies and procedures regarding access to all medical records that are designed to safeguard your privacy. These policies explain the circumstances under which your medical information may be released to parties outside the System. Our staff is educated about these policies and procedures and the appropriate use of medical information. Saint Francis has a Notice of Privacy Practices, available at its website or at any Saint Francis location.

Your Medical Record

As we provide your health care, we must keep records of your medical history, current conditions, treatment plan and all treatments given, including the results of all tests, procedures and therapies. Messages you send via MyChart may be made part of your permanent medical record at the discretion of Saint Francis, your doctor, or support staff. Once it is made part of your record, it will be accessible to current and future staff members of Saint Francis who are appropriately involved with your care, the payment for any services, necessary administrative work, and other disclosures as described in the Saint Francis Health System Notice of Privacy Practices. All electronic medical record content generated by Saint Francis is subject to state and federal laws governing the security and confidentiality of medical records.


You understand that messages will be routed to the appropriate department as necessary for handling, and therefore Saint Francis employees, other than your healthcare provider may be involved in addressing your request. Therefore use of MyChart may not be appropriate if there is sensitive information that you would only wish to discuss directly with your doctor.

You understand that you may receive messages, test results, and other communications delivered via MyChart. Therefore, you may not receive duplicate written reports of this information from Saint Francis. You understand that not all of your Saint Francis medical record may appear on your MyChart. Posting of information is at the discretion of your individual doctor(s) and providers.

As explained above, MyChart should never be used for urgent matters. Delays can occur based upon volume, availability of staff, and the complexity of your conditions. For all medical emergencies, immediately dial 911.

Internal Service Use

MyChart may be routed to other authorized caregivers within Saint Francis and its affiliates in order to facilitate a timely response to your request. All communications between you and your Saint Francis Health System health care team using MyChart are carried over a secure, encrypted connection directly into the Saint Francis electronic medical record. While you may receive an Internet e-mail message notifying you of new message in your MyChart Inbox, these e-mails will not contain any confidential medical information.

Proxy Access and MyChart

Parents/legal guardians of a minor patient and representatives appointed by an adult patient or by a court may request access to that patient's MyChart account. This type of access is known as proxy access. The proxy will provide documentation to verify their authority upon our request. There are exceptional situations in which a minor's consent may be required for access to his or her information. Adult patients without a legal representative and who are unable to consent to another's access of their MyChart information will need a legal guardian to establish MyChart access. Proxy access status for MyChart does not necessarily affect any other legal right the proxy may have to the principal's medical records or information.

Copyright and Trademarks

All content on Saint Francis Health System Web sites, including, but not limited to, text, photographs, graphics, button icons, images, artwork, names, logos, trademarks, service marks and data (the "Content"), in any form including the compilation thereof, are protected by U.S. and international copyright law and conventions. Except as set forth below, direct or indirect reproduction of the Content, in whole or in part, by any means, is prohibited without the express written consent of Saint Francis Health System.


This Web site uses encryption software. Once your medical information is received by us, it will be treated as confidential and given the same protection that all other Saint Francis medical records are given. However we cannot guarantee that information will be perfectly protected. Information will be transmitted over a medium that is beyond the control of Saint Francis Health System and its affiliates.

Assumption of Risk: You expressly assume the sole risk of any unauthorized disclosure or intentional intrusion, or of any delay, failure, interruption or corruption of data or other information transmitted in connection with the use of this service.

This site may contain links to web sites operated by other parties. The links are provided for your convenience only. We do not control such web sites and we are not responsible for the content and performance of these sites. The inclusion of links to other web sites does not imply any endorsement of the material on the web sites or any association with their operators. Saint Francis does not operate, control or endorse any information, products or services provided by third parties through the internet. Your use of such sites is at your own risk including, without limitation, any risk associated with destructive viruses.

No Warranties

This site and all other sites hosted by Saint Francis, and the content contained herein and therein, are provided on as "as is" basis. Saint Francis makes no representations or warranties of any kind, express or implied, as to the operation of its sites, or the content, products or services included therein. To the fullest extent permissible by applicable law, Saint Francis disclaims all warranties, express or implied, including, without limitation, implied warranties or merchantability, fitness for a particular purpose, title and infringement.


Participation in MyChart is voluntary. Users may request deactivation of an account by contacting their physician's office. Saint Francis reserves the right to deactivate or prevent a user's access to any or all of MyChart features at any time for any reason.


By using MyChart or any Saint Francis web site, we and you agree that the laws of the State of Oklahoma, without regard to principles of conflict of laws, will govern these Terms and Conditions of use and any dispute that might arise.
